By submitting this I hereby authorize Safehavenforwidowed (SHFW) to use my personal story, recipe, poem, etc. and testimonial in pursuit of their mission giving support to the widowed and non-widowed community worldwide.I understand that my story and testimonial may be used in a wide variety of materials, including newsletters, flyers, posters, brochures, advertisements, websites, social networking sites, and other print and digital communications.

I hereby waive the right to receive any payment for SHFW use of any of the material described above for any of the purposes authorized by this release. I also waive any right to inspect or approve finished copy or printed matter and other electronic media that may be used in conjunction therewith or to approve the eventual use that it might be applied.
